Start with the published eligibility criteria

• The child must be under 18 and reside in Alberta.
• The child must be a Canadian citizen or permanent resident.
• The person applying must be the child's guardian.
• Medical documentation must confirm a chronic qualifying disability or show that diagnosis is in progress.

Prepare the application documents

The FSCD application page states that an application is complete only when the application and supporting documents have been received. It lists proof of the child's citizenship or immigration status, medical documentation, and proof of guardianship when applicable.

Medical information should identify the child, the diagnosis or probable diagnosis, the relevant date and the professional supplying the information. Use the official page to confirm which regulated professionals and formats currently meet the requirement.

What happens after application

Disability Services reviews the package and may ask for more information, explain an ineligibility decision, or confirm eligibility and discuss next steps. Eligibility is followed by a conversation about the family's situation, needs, goals and possible supports.

Some Family Support Services may be put in an agreement before a fuller needs assessment. Additional services involve a caseworker assessment and, where applicable, an Individualized Family Support Plan.

Supports depend on the agreement and assessed needs

The official supports page lists navigation, coordination, counselling, childcare supports, some medical-appointment costs, respite, aides, certain disability-related costs, specialized services and transition planning among the possible supports. This is not a promise that every eligible family receives every listed service.

FSCD also identifies areas it does not cover, including educational needs, emergency or crisis needs, equipment covered through other programs, individual therapy and general financial benefits. Confirm the boundary for a proposed expense before committing money.

Questions to record for the FSCD worker

• Is the application complete, and is any document still outstanding?
• Which support need is being assessed and what additional evidence would be useful?
• What does the proposed agreement cover, for what period and under which claim process?
• Which receipts, invoices and employment records must the family retain?
• How can the family request a change, review a decision or appeal within the official timelines?

Use the written decision and agreement

ARH cannot decide FSCD eligibility, service level or reimbursement. Keep the written eligibility decision and signed agreement as the controlling record, and ask the assigned worker to clarify anything that is not explicit.

If the family disagrees with a decision, use Alberta's current resolve-concerns and appeal pages promptly because formal timelines may apply.
