Nitric oxide and peroxynitrite are not validated autism diagnostic biomarkers.</strong></p> <h2>Bottom line</h2> <p>A 2025 narrative review proposes studying nitric oxide and peroxynitrite in induced pluripotent stem cells and laboratory-grown brain organoids. The idea may help researchers investigate oxidative-stress pathways, but the paper does not validate a blood test, brain test or other clinical diagnostic test for autism.</p> <h2>What the paper proposes</h2> <p>Researchers can reprogram donated body cells into induced pluripotent stem cells and use them to grow three-dimensional brain organoids. These organoids are laboratory models, not miniature versions of an individual’s complete brain. The review proposes comparing nitric oxide and peroxynitrite measurements in cells and organoids derived from autistic and non-autistic participants as a possible step toward future biomarker research.</p> <h2>What the paper does not establish</h2> <ul> <li>It does not report a validated diagnostic cutoff.</li> <li>It does not establish sensitivity, specificity or accuracy in a prospective clinical population.</li> <li>It does not show that nitric oxide or peroxynitrite can diagnose an individual child or adult.</li> <li>It does not replace developmental history, observation and clinical assessment.</li> </ul> <h2>Current clinical context</h2> <p>Current guidance describes autism diagnosis as a clinical process based on development and behaviour; there is no medical test such as a blood test that diagnoses autism. A large systematic review of autism biomarker research found extensive heterogeneity, small studies and inconsistent replication, with no biomarker sufficiently established to guide autism clinical trials at the time of that review.</p> <h2>What families should know</h2> <p>This is promising laboratory research, not an actionable test. Families should be cautious about commercial products or services claiming that oxidative-stress markers can confirm or rule out autism.
