ITP (Individualized Transition Plan)

What it means

Transition planning recorded in a US student's IEP to support movement from school to postsecondary education, employment, and, where appropriate, independent living. Under IDEA, transition services must be in effect no later than the first IEP when the student turns 16, or younger if the IEP team or state rules require it.
