- How is progress measured and explained in plain language?
- How can goals or the service plan be changed?
Availability, costs and administration
- Is the provider currently accepting new clients?
- Estimated time to intake and time to regular service
- Assessment, session, report, cancellation and travel fees
- Public funding, insurance or direct billing accepted
- Referral, diagnosis or other intake requirements
- What happens if the usual practitioner is unavailable?
